they can use to develop their children’s confidence and improve their school experience.
This page is designed to provide you with
all needed registration information and required forms for the upcoming
school year. To register your child, you must bring all needed forms
and required documents to Central Avenue Elementary School in person.
I. Proof of Residency in School Zone:
resident parent or guardian admitting a pupil to an Osceola County
School shall produce at least two (2) forms of residential
II. Proof of Physical Examination:
physical examination must have been completed within the last twelve
(12) months prior to day of enrollment. Students having attended a
Florida school are exempt when providing documented evidence of a
physical from that Florida school.
All student immunizations must be up to date and on Florida Form 680.
IV. Proof of Date of Birth:
requires that a child must be five years old on or before September 1
of the ensuing school year to be eligible to enroll in kindergarten. A
child must be six years old on or before September 1 of the ensuing
school year to be eligible to enroll in first grade.
